Cystic Acne at Home: Calm First, Then Correct

Cystic acne is the deep, painful kind: large, firm bumps under the skin with no head, that last for weeks and leave the worst marks and scarring of any breakout. It needs a different approach from surface acne, because treating inflamed deep skin with strong actives makes it worse. Calm first, then correct.

What makes it cystic

A regular breakout is a blocked pore near the surface. A cyst is the same blockage much deeper, with inflammation spreading into surrounding tissue. That depth is why it hurts, why it lasts, why there is no head to extract, and why it damages collagen on the way out, which is what leaves an indented scar behind.

The rule: calm before correct

Putting a retinoid or a strong acid on actively inflamed deep skin adds irritation to inflammation. On skin that marks easily, that is the recipe for the darkest, most stubborn marks. So the order is fixed: settle the inflammation first, then treat what it left.

Phase 1: the first two weeks

  1. Morning and night: Exfoliating Cleanser, pea-sized, 30 to 60 seconds, gently. No scrubbing.
  2. Morning and night: Clarifying Gel, 2 pumps over the whole face, dried 1 to 2 minutes. This treats the active cysts and the congestion forming the next ones.
  3. Morning: Refining Crème, 4 pumps, for SPF.

No retinoid yet. No acids beyond the cleanser. Do not touch the cysts. Hands off entirely.

Phase 2: once inflammation has settled

When the active cysts are no longer tender and swollen, usually after two weeks on phase 1, add Resurfacing Crème at night: patch test, ease in every other night for a week, then 14 nights on and 3 off, 5 pumps in a thin even layer. This fades the marks and, over 3 to 6 months, softens the indented scarring as collagen rebuilds.

If new cysts flare while you are on Resurfacing Crème, keep the cleanser and gel going, skip the crème on the inflamed area until it settles, then resume.

When a routine is not enough on its own

Cystic acne that is widespread, constant, or clearly tied to your cycle often has a hormonal or internal driver that topicals do not reach. A routine still matters, because it manages the surface and fades the marks, but it may need prescription support alongside it. That is a both-and, not an either-or. Can I pop a cyst?

No. There is nothing at the surface to remove. Pressure drives the inflammation deeper, spreads it, and damages the collagen that decides whether you get a mark or a permanent scar.

Why can't I start Resurfacing Crème straight away?
